After completing the demo, fill out and submit the Pre-Approval Form using the link provided below to receive a quote.
4.
Purchase your qualifying 3Shape product within two (2) weeks of receiving pre-approval to remain eligible for the rebate.
5.
Once your purchase has been validated, your rebate will be processed and issued according to the program terms and conditions.
